Sylvester’s My Wellness Check Uses Artificial Intelligence to Transform Cancer Care

By adding artificial intelligence to the My Wellness Check platform, Sylvester Comprehensive Cancer Center is reshaping how it supports patients—by listening to them.

Three years after Sylvester Comprehensive Cancer Center launched My Wellness Check, the platform is now entering its next evolution — using artificial intelligence to enhance clinical decision-making and survivorship care. 

Developed by the Sylvester Survivorship and Supportive Care Institute, My Wellness Check is a secure, electronic medical record-integrated questionnaire that patients complete before their appointments. The system assesses emotional and physical well-being, including anxiety, pain and fatigue, quality of life, nutrition and practical needs such as transportation and childcare.  The questionnaires are scored in real time, and if the patient reports elevated symptoms or concerns, their treatment teams are notified. 

“We designed My Wellness Check to give patients a voice in their care,” said Frank Penedo, Ph.D., associate director for population sciences and director of the Sylvester Cancer Survivorship and Supportive Care Institute (SSCI). “Patients appreciate being asked about their well-being and knowing that their responses are being used to guide care. Now, with AI, we’re taking that to the next level — using machine learning to identify risk patterns and tailor interventions more precisely and get patients to the right level of care at the right time.”

AI Meets Empathy

The next evolution of My Wellness Check involves artificial intelligence algorithms that analyze the data collected over time and how these measures relate to patient outcomes. These algorithms will help clinicians detect subtle changes in symptoms, flag high-risk patients and recommend personalized care pathways.

For example, if a patient’s responses indicate increasing distress or unmanaged pain, the system can alert the care team and suggest evidence-based interventions, such as referrals to social work, nutrition or psychosocial oncology.

“AI allows us to be proactive rather than reactive,” said Jessica MacIntyre, D.N.P., M.B.A., APRN, clinical operations lead at SSCI. “It’s about using technology to deliver compassionate, timely care that meets patients where they are.”

The tool is grounded in research findings, such as those published in Cancer (2022), which show that routine symptom monitoring improves outcomes and enhances patient-provider communication. My Wellness Check is integrated into Sylvester’s electronic health record system for seamless access prior to and during appointments, creates visual dashboards to help clinicians address top patient concerns and is accessible in English, Spanish and French on smartphones, tablets or computers. This approach aligns with broader trends in digital health and precision medicine, where AI is increasingly used to support clinical workflows and improve outcomes.

Empowering Survivors and Enhancing Outcomes

Beyond active treatment, My Wellness Check plays a vital role in cancer survivorship. As more patients live longer after diagnosis, addressing long-term physical and emotional challenges becomes essential. The AI-enhanced platform will help survivorship teams monitor late effects, track recovery trajectories, and deliver personalized support for issues like fatigue, fear of recurrence and return-to-work concerns.

A Model for the Future

Sylvester’s My Wellness Check is already drawing attention as a model for value-based, tech-enabled cancer care. By combining patient-reported outcomes with AI-driven insights, the platform exemplifies how academic medical centers can lead in digital innovation. 

A deeper insight into the technology’s capacity will take place Nov. 7-9, as members from Sylvester and the University of Miami will present their early work on the topic at the Cancer Center Informatics Society Fall Symposium in Miami, Florida. 

‘My Wellness Check is more than a tool — it’s a philosophy,” Dr. Penedo said. “By integrating clinical and patient-reported data with AI-powered risk assessment and tailored interventions, we’re able to deliver smarter, evidence-based care that truly improves patient outcomes.”
