For the 25th time, Bascom Palmer Eye Institute has been recognized by U.S. News & World Report as the nation’s top ophthalmology program, a distinction that reflects decades of leadership in patient care, research and medical education.

As Bascom Palmer enters a new chapter of leadership, Pradeep Ramulu, M.D., M.H.S., Ph.D., professor and chair of the Department of Ophthalmology at the University of Miami Miller School of Medicine, director of Bascom Palmer Eye Institute and the Kathleen and Stanley J. Glaser Chair in Ophthalmology, and Eduardo Alfonso, M.D., professor of ophthalmology at the Miller School, who served as director of Bascom Palmer Eye Institute and chair of the Department of Ophthalmology from 2007 to 2026, spoke about the role academic medicine plays in advancing vision care and what the future holds for patients.

What does academic medicine mean for our patients? 

Dr. Ramulu: Bascom Palmer’s rise to national and international prominence speaks to its deep commitment to excellence in clinical care, vision research and medical education, which I observed firsthand as a fellow here in 2006 and 2007.

Dr. Alfonso: Academic medicine at Bascom Palmer focuses on providing the most advanced vision care for our patients, while educating the next generation of medical professionals and conducting high-impact clinical and scientific research.

Academic health care institutions like Bascom Palmer also contribute to society in many ways. For instance, our screening programs help South Florida children and adults access appropriate vision professionals in their communities. In addition, our clinical and scientific research hold the promise of powerful advancements in ophthalmology.

What distinguishes your program’s approach to caring for patients with complex conditions? 

Dr. Ramulu: Excellence in clinical care, the discovery of transformative new knowledge and training the next generation of physicians and scientists are driving our ability to diagnose and treat patients with complex visual conditions.

Dr. Alfonso: Our faculty have decades of experience in treating blinding diseases and are involved at the highest level of national and international developments in their specialties, which include corneal diseases, retinal diseases, glaucoma, pediatric ophthalmology, neuro-ophthalmology, ocular oncology and ophthalmic plastic and reconstructive surgery. Our faculty continually learn from each other, honing their skills and continuing to develop novel techniques for the benefit of our patients.

What advances in treatment, technology or clinical care are having the biggest impact on patients right now? 

Dr. Ramulu: Glaucoma care has changed dramatically throughout the decades and the number of treatment options continues to increase. Today, AI provides new opportunities to predict who will develop this blinding disease and how quickly the disease is likely to advance. That will help our clinicians deliver state-of-the-art care to our patients.

One of the most exciting developments in ophthalmology is our growing ability to deliver increasingly personalized care. Advances in imaging, genetics, artificial intelligence and data science are helping physicians detect disease earlier, better understand how conditions progress and tailor treatments to individual patients. At Bascom Palmer, our clinician-scientists work closely with researchers and collaborators across the University of Miami to translate these discoveries into better outcomes for patients.

Dr. Alfonso: Our institute is conducting high-impact research in preventative therapies, such as identifying biomarkers of disease and developing gene therapy, cellular therapy and advanced surgical procedures. We are also developing new technologies in biomaterials, drug delivery vehicles and diagnostic imaging instruments in collaboration with University of Miami engineering and data science professionals. For instance, glaucoma patients face a higher risk of falling due to their poor vision. It may sound simple, but we found that improving the lighting in the home reduced the risk of falling and improved patient safety.

Patients benefit from access to clinical trials, advanced diagnostic technologies and emerging therapies that may not yet be widely available. Just as important, our multidisciplinary teams work together to provide coordinated care that helps patients navigate complex conditions more efficiently and with greater confidence.

Dr. Alfonso: Bascom Palmer has long been known for leadership utilizing technology in providing clinical care. The pandemic kicked our telemedicine, virtual care and remote monitoring programs into high gear. Ophthalmology lends itself to telehealth or virtual care because digital images of the eye can be transmitted to our specialists from almost anywhere. We are also developing home testing and monitoring devices so patients will be able to check their intraocular pressures or visual fields.

What is the one initiative currently underway in your specialty that you believe will define the next chapter of patient care here? 

Dr. Ramulu: We have a great opportunity to advance glaucoma care through multiomics, an integrated approach to understanding the genetic, biologic and environmental factors related to vision diseases. This is an area where Bascom Palmer is uniquely positioned to invest in the infrastructure that will contribute to our ongoing global leadership in vision care.

Dr. Alfonso: Artificial intelligence (AI) will continue to play an increasingly important role in the diagnosis and treatment of blinding conditions. I am confident that Bascom Palmer will continue to lead the way in this field, while serving as a beacon for hope for patients and families from all walks of life who need excellent vision care.
